Services - Individual

The AWARDS Services - Individual module is used to maintain and review individual services records, including those for contacts and progress notes, service plans,  and service coordinators.

Required Permissions

Access to the Services - Individual module require the following permissions:

Display Any Chart Records Buttons
Display Chart Records Services - Individual Button

Additional permissions required to use the individual features within the Services - Individual module are discussed in detail in the Online Help sections for each of those features.

  NOTE: Permissions are assigned using the Permissions Maintenance feature.  If you do not have access to that feature and need the permission listed here, please contact your supervisor or your local Help Desk for assistance.

Module Features

The Services - Individual module is comprised of the following features.  Click a link below to access the Online Help pages for the corresponding feature.

AMENDMENTS - Learn to record client requests for amendments to their health records and to view amendment data reports.

ASSESSMENT DATA - Learn to enter assessment data for scheduled events and to view assessment data reports.

BEHAVIORS - Learn to record a client's target and replacement behaviors, as well as sub-records for associated tracking of those behaviors.

BUDGETS - Learn to record and maintain client budget records, including line items, as well as to view detailed budget/line item reports.

CHARTING TIMETABLE - Learn to enter, update, and delete charting timetable records and to view charting timetable reports.

CLIENT ALERTS - Learn to record and maintain client alerts information that can then be viewed by all staff within Client Search results.

CLIENT FILE CABINET - Learn to access and work with the File Cabinet available for individual clients here and on client face sheets.

CONTACTS LOG / SUPPORTIVE SERVICES CHECKLIST - Learn to enter, update, and delete contacts log records and to view contacts log reports.

OMH PCS 2019 - Learn to generate the 2019 Patient Characteristics Survey.  * This functionality is configured to meet requirements for programs in New York state.

PLANS AND REVIEWS - Learn to fill out PlanBuilder plans and reviews, and to view read-only reports of plan data.

PROGRAM STATUS - Learn to maintain records of client status separate from program history information.

PROGRESS NOTES - Learn to enter, update, and delete progress notes and to view progress note reports.

RES/DAY HAB DAILY CHECKLIST - Learn to complete and view daily checklists.

SERVICE COORDINATORS - Learn to update service coordinator assignment information and to view service coordinator assignment reports.

SERVICE PLANS - Learn to enter and update service plans and to view service plan reports.

SERVICE REFERRALS - Learn to enter and update consumer referrals.

TOXICOLOGY INFORMATION - Learn to record client toxicology information and to view read-only toxicology reports.

TRANSPORTATION - Learn to record ride schedules, expected passengers, rides completed, and transported passengers, as well as to view transportation reports.

UTILIZATION REPORTS - Learn to view lapse audit and Medicaid Services utilization reports.

VPAC ASSESSMENTS - Learn to use this optional add-on service to access and complete Virtual Psych Assessment Center assessments for the individuals you serve.

Additional Services - Individual Reports 

The following reports, while not located directly within the Services - Individual module itself, can be a useful tool for analyzing services-related information.

Caseload Activity Report - Used to generate reports of key info for a worker's entire caseload. 

E-Signable Records ReportBuilder - Used to generate reports of records configured with e-signatures, showing which have been signed, still need signing, and more.

Service Contacts ReportBuilder - Used to generate custom reports of individual and group services.

Review & Refresh - Documenting After Discharge (33 min) * - December 2016

Learn different ways users can document information post-discharge with a wide variety of AWARDS features, including use of roster archives, post-discharge notes, follow-up notes, group notes, correcting service coordinators, charting timetable, backdating discharges, readmitting clients that were previously discharged, and undoing discharges.
