SERVICE PLANS

The Services - Individual module's Service Plans feature is used to enter and update consumer service plans, as well as to view service plan reports. There are several service plan formats available for use within AWARDS, with the two most commonly used being:

Single Goal Service Plans

Multiple Goal Service Plans

Please verify the type of service plan used at your agency to make sure you refer to the appropriate instructions.

TIP: Other service plan formats may be used by specific program types. For more information on use of those formats (for example, life area treatment plans), please refer to the instruction sheets found here.

IMPORTANT! Keep in mind that agencies where AWARDS is configured with the new PlanBuilder functionality, the Plans and Reviews feature may be in use in place of the Service Plans feature for specific individuals and/or programs.

Required Permissions

Use of the Service Plans feature requires the following permissions:

Program Chart Access
Display Any Chart Records Buttons
Display Chart Records Services - Individual Button

NOTE: Permissions are assigned using the Permissions Maintenance feature. If you do not have access to that feature and need a permission listed here, please contact your supervisor or your local Help Desk for assistance.
