Viewing a Basic Services Delivery Report

The Services Delivery Report, available from within the AWARDS Reports module, contains individual detail and summary information for service contacts that took place during a specified date range.  To view a Supportive Services Delivery Report, complete the following steps:

IMPORTANT! The Supportive Services Delivery Report looks at individual services recorded in each contact record using the supportive services checklist.  To view a version of the report that includes data on the "Primary Problem Area" service type rather than on in the individual services provided, please use the Contacts Log Report feature, also in the Reports module.

    1. From the navigation bar, click Reports (or the Reports icon).  The Reports index page is displayed.

    1. Click the Program drop-down arrow and select the individual program for which the report is to be run, or select a group of programs using one of the available "All" or "Custom Grouping" selections.
    2. If you previously bookmarked the Services Delivery Report it is displayed on the reports index by default; otherwise, from the left-hand menu bar, click All under "Show Reports."
    3. Use the navigation features in the bottom-right corner of the reports index to locate the Services Delivery Report, or enter "delivery" in the Search field in the upper-right corner.
    4. Click Services Delivery Report from the reports index. The Services Delivery Report page is displayed.

    1. Only information regarding services provided during the date range entered in the Start Month/Year and End Month/Year fields on this page will be included in the report.  Make changes to the default date range as necessary by clicking the drop-down arrows and selecting the correct start and end months and years.
    2. Click one of the service type radio buttons to indicate what type of information is to be included in the report.  Available options are:

  Overview of Services Provided - When this option is selected, the report will include information on the number of services per client, number of clients served, and the number of services provided per client per day, as well as the number of progress notes written for each service type.

  Note Days vs Services by Coverage - When this option is selected, the report will be  generated in the form of a service coordinator documentation activity report for a selected worker or all workers.  This report will include information on those consumers each service coordinator has written a note for during the selected time period, as well as the number of note days and ISP note dates for each of those consumers.  This report will also contains the total number of progress notes, group notes, referral tracking notes, consumers, attendance, and applicants for each service coordinator or the selected service coordinator.

    1. If the report should only include information pertaining to service plan linked notes rather than all progress notes, click the Report Service-Plan-Linked Notes Only? checkbox.
    2. Click CONTINUE

If the Overview of Services Provided option was selected in step 7, the read-only services delivery report is displayed on the Services Delivery Report page.  The process of viewing a services delivery report is now complete.

If the Note Days vs Services by Coverage option was selected in step 7, the Service Coordinator Documentation Activity Report Worker Selection page is displayed.  Continue with step 10.

    1. Click the Worker drop-down arrow and select the worker/service coordinator for whom the report is to be viewed.  The default value is "All Workers."
    2. Click CONTINUE.  The service coordinator documentation activity report is displayed on the Primary Service Coordinator Documentation Activity Report page.

The process of viewing a services delivery report is now complete.
