The AWARDS Outcomes module is used to monitor agency operation in areas such as goal outcomes through the viewing of read-only reports. It is also used to create and maintain forms used to collect data throughout AWARDS.
Required Permissions 
Permissions required to use the Outcomes module are as follows:
Display Any Chart Records Buttons
Display Chart Records Outcomes Button
Additional permissions required to use the individual features within the Outcomes module are discussed in detail in the Online Help sections for each of those features.
NOTE: Permissions are assigned using the Permissions Maintenance feature. If you do not have access to that feature and need a permission listed here, please contact your supervisor or your local Help Desk for assistance.
Module Features 
The Outcomes module is comprised of the following features. Click a link below to access the Online Help pages for the corresponding feature.
ASSESSMENT DATA - Learn to enter assessment data for scheduled events and to view assessment data reports.
DASHBOARD - Access a "dashboard" of graphs and aggregate data that will help meet basic reporting and oversight needs.
FORMBUILDER REPORTBUILDER - Learn to generate ad hoc reports of FormBuilder form data.
SERVICE UTILIZATION REPORTS - Learn to monitor service utilization through the viewing of read-only services delivery reports.
Additional Outcomes Reports 
The following reports, while not located within the Outcomes module itself, can be a useful tool for analyzing outcomes-related information.
Discharge Levels Report - A saved report format ("Foothold Report - Discharge Levels") in the Demographics ReportBuilder. Reports on outcome information.
Goal Outcomes Report - Located in the Reports module. Used to assess goal outcomes.
Hospitalization Rate Report - Located in the Reports module. Used to monitor client hospitalization rates.
