Viewing a Goal Outcomes Report

A sortable read-only outcomes-related Goal Outcomes Report is available from within the AWARDS Reports module.  To view this report, complete the following steps from the AWARDS Home screen:

    1. From the navigation bar, click Reports (or click the Reports icon).  The Reports index page is displayed.

    1. Click the Program drop-down arrow and select the program or group of programs for which the report is to be viewed.

IMPORTANT! This report is designed for use by programs whose service plans use goal categories.  IRA service plans and others without goal categories will not return meaningful data in this report.

    1. If you previously bookmarked the Goal Outcomes Report it is displayed on the reports index by default; otherwise, from the left-hand menu bar, click All under "Show Reports."
    2. Use the navigation features in the bottom-right corner of the reports index to locate the Goal Outcomes Report, or enter "goal outcomes" in the Search field in the upper-right corner.
    3. Click Goal Outcomes Report from the reports index.  The Goal Outcomes Report page is displayed.

    1. The Start Month/Year and End Month/Year options on this page are used to control which goals (or objectives in the case of programs using the single-goal service plan format) are included in the report.  Specifically, goals are only included in the report if they:

  are for clients on the program census during part or all of the date range

  are on plan reviews with due dates during the date range (and during the client's program episode)

  have had an outcome selection made for them on the plan reviews

Make changes to the default date range as necessary by clicking the drop-down arrows and selecting the correct start and end months and years.

    1. Click CONTINUE. The goal outcomes report is displayed on the Goal Outcomes Report page.

The contents of this read-only report include statistical information on the number and percentage of evaluated goals on plan reviews due during the selected date range.

The process of viewing a goal outcomes report is now complete.
