Viewing a Hospitalization Rate Report

A sortable read-only outcomes-related Hospitalization Rate Report is available from within the AWARDS Reports module.  To view this report, complete the following steps from the AWARDS Home screen:

    1. From the navigation bar, click Reports (or click the Reports icon).  The Reports index page is displayed.

    1. Click the Program drop-down arrow and select the program or group of programs for which the report is to be viewed.
    2. If you previously bookmarked the Hospitalization Rate Report it is displayed on the reports index by default; otherwise, from the left-hand menu bar, click All under "Show Reports."
    3. Use the navigation features in the bottom-right corner of the reports index to locate the Hospitalization Rate Report, or enter "hospitalization" in the Search field in the upper-right corner.
    4. Click Hospitalization Rate Report from the reports index. The Hospitalization Rate Report Settings page is displayed.

    1. Only those hospitalization records falling within the date range selected using the Start Month/Year and End Month/Year options on this page will be included in the report.  Make changes to the default date range as necessary by clicking the drop-down arrows for each option and selecting the correct start and end months and years.
    2. Click the Diagnosis drop-down arrow and make a selection to indicate which hospitalization records are to be included in the report based on consumer diagnosis.  Available selections are:

  Both Axis I and No Axis I - When selected the report will include records for those consumers with or without Axis I diagnoses.

  DSM Axis I - When selected the report will only include records for those consumers with Axis I diagnoses.

  No Axis I - When selected the report will only include those records for consumers without Axis I diagnoses.

    1. Click the Eligibility drop-down arrow and make a selection to indicate which hospitalization records are to be included in the report based on NY/NY eligibility. 

  NOTE: This option is not available for non-NY/NY programs.

Available selections are:

  Both NY/NY and Non-NY/NY - When selected the report will include records for those consumers with or without NY/NY eligibility.

  Eligible NY/NY Only - When selected the report will only include records for those consumers with NY/NY eligibility.

  Ineligible NY/NY Only - When selected the report will only include those records for consumers without NY/NY eligibility.

    1. Click the Program Duration drop-down arrow and make a selection to indicate which hospitalization records are to be included in the report based on the consumers' length of stay at the selected program.  Available selections are:

  All - When selected the report will include records for all consumers, regardless of their length of stay in the selected program.

  Up to 6 Months - When selected the report will only include records for those consumers with a length of stay at the selected program of up to six months.

  6 Months - 1 Year - When selected the report will only include those records for consumers with a length of stay at the selected program between six months and one year.

  1-2 Years - When selected the report will only include those records for consumers with a length of stay at the selected program between one and two years.

  2 Years or More - When selected the report will only include those records for consumers with a length of stay at the selected program of two years or more.

    1. If the report should be limited to records for psychiatric hospitalizations, click the Psych. Hospitalizations Only? checkbox.
    2. Click CONTINUE.  The hospitalization report is displayed on the Hospitalization Rate Report page.

The contents of this read-only report are based on the selections made on the previous page; however, all reports include a hospitalization rate summary, as well as detailed hospitalization rate information by program.

The process of viewing a hospitalization report is now complete.
