Are notifications sent when a plan/review is ready to be signed?

Yes.  An internal audit message is automatically sent to workers who are required to sign a plan/review before other signatures can be applied, based on the rules configured for the signature line in the PlanBuilder(Notifications require that the signature type for the signatures be set to "Employee.")

Are plans and reviews included on charting timetable reports?

Data related to plans and reviews is included on the Charting Events ReportBuilder, but NOT on the Charting Timetable Report.

Can clients with AWARDS logins e-sign plans/reviews on their own?

No, at present clients must have the assistance of a staff member in order to e-sign their own plans/reviews.  Future development will enable them to e-sign plans/reviews while logged into AWARDS as they currently can for legacy service plans; however, that development is currently in progress and not yet available.

Can I use plans and reviews for a pending client whose intake has been processed but who is not admitted?

Yes.  If the plan is configured to autoschedule based on admission date, you will have to manually schedule the initial items on the Plans and Reviews index but, once scheduled, you can enter data into plans and reviews.

How can I start a brand new plan for an individual if I need to start fresh with a blank plan for any reason?

To do so, schedule a new initial plan for the individual.  For instructions on the scheduling process, review to the "Schedule and fill out a new plan/review" bullet under Working with Plans / Reviews.

How do I change a plan that has been put into effect?

To make changes to a plan that has an effective date, open the plan in Edit mode, and then navigate to the Attributes portion of the plan.  Click the delete icon (blue x) for the effective date, and then click Save.  This re-opens the plan to changes.

IMPORTANT! The option to delete the effective date is only available if no plan linked notes and/or group notes have been written for the plan.

How do I create a plan amendment?

Create a new plan where the Due Date is after the current plan's Due Date, but before the next scheduled plan review.  Users with access to the PlanBuilder can configure a field to note this is a plan amendment.

How do I delete a plan or review?

A plan/review can only be deleted if there is currently no effective date set for it, it has not been e-signed, and the content of no later plan/review has been copied from it.

Keep in mind that if your agency has the optional "Plans and Reviews Deletion" permission feature turned on, you must have that permission in order to delete unlocked plans. 

  NOTE: Permissions are assigned using the Permissions Maintenance feature.  If you do not have access to that feature and need the permission noted here, please contact your supervisor or your local Help Desk for assistance.

How do I delete or backdate the Effective Date on a plan that has Service Plan Linked Notes?

To delete or backdate the Effective Date on a plan that has Service Plan Linked Notes, complete the following steps:

    1. Use the Progress Notes feature to temporarily change the note type of the Service Plan Linked Notes to "General Chart Notes" (or something similar).  This will unlink the notes from the plan.  Base the specific list of notes to be changed on the following:

  If the Effective Date needs to be deleted - Change the note type for all Service Plan Linked Notes dated between today and the current Effective Date on the plan.  

  If the Effective Date needs to be backdated - Change the note type for all Service Plan Linked Notes data between today and the desired backdated Effective Date.

  TIP: If the Service Plan Linked Notes have been electronically signed, the e-signatures must be removed before the note type can be changed. For more information, please refer to the Removing Electronic Signatures - Instruction Sheet.

    1. Once the associated Service Plan Linked Notes have been unlinked, the Effective Date field opens for editing and the date can be deleted or backdated as needed.  Do so from within the Plans and Reviews feature.

  NOTE: Some agencies are configured to restrict who can enter Done Dates for charting events.  If your agency is using this restriction you must have the Plan Effective Date Data Entry permission in order to delete or backdate the Effective Date.  Permissions are assigned using the Permissions Maintenance feature.  If you do not have access to that feature and need the permission noted here, please contact your supervisor or your local Help Desk for assistance.

    1. Return to the Service Plan Linked Notes that you unlinked from the plan in step 1, and for each:

  Change the note type back to Service Plan Linked Note.

  Re-apply your electronic signature to the note, if applicable.  

The process of deleting or backdating an Effective Date on a plan with Service Plan Linked Notes is now complete.

How do I specify which job title I want to e-sign a plan/review with if I have more than one?

Job title information is automatically taken from your Staff Information record in the Human Resources module and displayed in a field to the left of the PIN in the signing dialog box.  If no job title is found it will instead display "<Insert Job Title to be saved with Signature>."  Verify that the correct job title is in place, or overwrite the default title or placeholder with the correct value.  If the "<Insert Job Title...>" value is left in place, no title is added to the signature.

How do I unlock a signed record?

At the time a record is electronically signed it is locked and can no longer be edited.  However, we understand that in rare instances staff utilizing electronic signatures may not be careful in confirming that they are signing the right document for the right client before proceeding, and that it may occasionally be necessary to correct such errors.  As a result, we have made available to agency staff functionality that can be used to remove electronic signatures from specific records at their discretion.  This functionality is only available upon request and must be turned on behind-the-scenes by your Senior Project Manager or the Foothold Help Desk.  For more information, please refer to the Removing Electronic Signatures - Instruction Sheet.

What do the dates on a plan/review's Attributes tab mean?

  Due Date - The date when the plan is due to be completed.

  Done Date - The date data entry on the plan was completed.  The Done Date does NOT lock the plan from further edits, nor does it speak to any other areas in AWARDS.

  Effective Date - The date the plan goes into effect and services can be entered/associated with the plan (such as Service Plan Linked progress notes).  Entering an Effective Date finalizes and locks the plans from further edits.

What's the difference between a plan's Done Date and its Effective Date?

Entry of the Done Date indicates that the plan was completed, but does NOT put the plan into effect or lock the plan from further edits.  The Effective Date finalizes and locks the plan, preventing further edits (aside from the Signatures tab where signers are designated, and the application of electronic signatures, if the plan has been set to use them.)  When an Effective Date is entered it will automatically populate the Done Date field as well, if a Done Date has not already been specified.

  TIP: You cannot enter an Effective Date that falls on or before the date of an existing linked records (service plan linked notes or linked group notes).  If such a date is entered, an error message is displayed with the first available date.

  NOTE: Effective and Done dates cannot be more than 60 days in the future.

When I run the ReportBuilder for One Row Per "Signature Action," why are some rows missing a lot of data?

In order for all of the necessary information to be collected for a signature row on the Plans and Reviews ReportBuilder, the signatures tab of the plan/review must have been saved in data entry mode to reflect any employee selections (even if it is only to accept the default values) to apply to the configured signature lines.  If that step has not been taken, large portions of the signature action row for employee signatures on the ReportBuilder will be blank.  Once the signature information has been saved for a plan/review, that plan/review's signature action rows on the ReportBuilder will fully reflect each configured signature. 

  NOTE: In Plans and Reviews data entry mode any employee signatures where there is a default signer role selected that has not yet been saved is appended with "([signer role] selected)" in order to help you identify that the page needs to be saved.  Once the save is complete, that temporary label is no longer displayed.

When I run the ReportBuilder for One Row Per "Signature Action," why are there more rows than there are configured signatures?

Initially - after the plan/review's signatures page has been saved but before any other action has been taken for those signatures - the report will show you one row per configured signature.  The "Signature Action" data variable for those rows (when included in the report) will be blank, indicating that no one has yet signed (or refused to sign. if applicable), and that therefore action may be necessary.

Once a signature action is taken for a given signature (for instance a refusal/unavailability is indicated or a signature is applied), the "configuration row" of the report for that signature is replaced with information for that action.  Any additional actions taken on that same signature are recorded as separate rows in order to fully reflect all actions taken.  So, for example, if I have one configured signature on my plan, a refused/unavailable action is taken on that signature, and later it is signed, there will be two rows for it in the ReportBuilder - one to reflect the refusal, and the other to reflect the signature.

When there are multiple signatures on a record, which date populates "Locked by E-Sign Date" on the ReportBuilder?

If multiple electronic signatures have been applied to a record, the ReportBuilder's Locked by E-Sign Date data variable populates with the date on which the first of those signatures was applied.  It is that date on which the record was locked from further edits.

When will workers be notified that a plan is ready to be e-signed?

When the plan's signatures page is saved in data entry mode for the first time, an automatically generated AWARDS message is sent to any employee(s) configured to sign the plan as part of the first order level.  If there is more than one order level for signing, messages are sent out to the employee(s) in subsequent levels as signing requirements are met in each previous level.  If a worker is configured to sign a plan but has not yet received a notification message, he/she will see the message "Other Signatures Required Before Signing is Available" on the plan instead of a signing link.  For more information on signing order levels and workflow, click here.

  TIP: A corresponding Ready for E-Signature Notification option is available under Notifications for all users who have the Receive External Email Notifications permission assigned to them under System Setup > Permissions Maintenance.  Users with this permission can receive external email notifications when an internal audit message is set to them within AWARDS letting them know when a note is available for signing.  In order for those notifications to be sent, the Notifications feature must be configured.

Who is listed in the "Reviewer" selection list when assigning plans and reviews?

Employees are included in the "Reviewer" selection list when one or more of the following criteria are met:

  The employee is in one of the following user groups:  "Direct Care Staff," "Executive Officer," "Entitlements Staff," "Housing Reception Desk," "Intake Staff," or "Direct Care Program Director.

  The employee has the "Include in Direct Care Staff List" permission assigned under Permissions Maintenance.

  The employee is the primary worker for the client in question, as specified under Service Coordinators.

Why am I unable to edit a plan's due date?

If the plan is locked, the plan's due date is also locked unless you have the "Plans and Reviews - Modify Due Date on Locked Plans" permission.  If you have that permission you can change the due date to any date between the previous plan's due date and the subsequent plan's due date.

If the plan is unlocked and you're unable to edit the due date, your agency likely has the optional "Plans and Reviews - Modify Due Date on Unlocked Plans" permission requirement turned on.  If so, you must have that permission in order to change the plan's due date.

Why are some fields/options available on one plan but not another?

The individual who created the plans and reviews using the PlanBuilder feature may have specified that a field/option be conditional.  In such cases it only displays on the plan or review when an associated question is answered in a specific way.

Why can I change the default signer for the e-signatures on some plans and reviews and not others?

When signature lines with a signer type of "Employee" and a default signer role are configured using the signature configuration settings in the PlanBuilder feature, there is an option available to Lock Employee Drop-Down.  If that option is set to "yes" for the signature line, the worker listed as the default signer on the plan cannot be changed when entering or signing the plan.  If you can change the default signer for some plans and not others (for instance in program A but not program B), it is an indication that that this lock option was set differently between those program types.

Why can't I copy a client's plan data over to the next plan review?

If the option to copy plan data into the next plan review is not available, it may be due to a change in plan templates that was made for the program type in the PlanBuilder.  Plan data cannot be copied between plans using different templates.  If the client's plan index contains two different PlanBuilder plans for the plan review and its previous plan, users cannot copy over the plan data.

Why can't I edit or delete a plan's Effective Date?

There are two main reasons why you may not be able to edit or delete the Effective Date on a plan in the Plans and Reviews feature:

  The Effective Date is locked from edits if there are Service Plan Linked Notes associated with that specific plan.  To identify whether Service Plan Linked Notes exist, click the Attributes tab of the plan while in editing mode.  If Service Plan Linked Notes exist, the following note is displayed below the Effective Date field: "Effective Date locked due to plan linked progress notes and/or group notes."

  In the event that there are no Service Plan Linked Notes associated with the plan, the Effective Date may be locked from edits if a subsequent plan review has been created and also locked with an Effective Date.  The Effective Date on that subsequent plan should be removed in order to edit or delete the Effective Date on the previous plan.

  NOTE: Some agencies are configured to restrict who can enter Done Dates for charting events.  Agencies using this restriction need to assign users the Plan Effective Date Data Entry permission in order for them to add/edit a plan's Effective Date.

Why can't I enter an Effective Date on a plan?

Some agencies are configured to restrict who can enter Done Dates for charting events.  If your agency uses this restriction you must have the Plan Effective Date Data Entry permission in order for you to add/edit a plan's Effective Date.

Why can't I enter an Effective Date or Done date in the future?

Effective Date and Done Date cannot be dated for more than 60 days in the future.

Why can't I write Service Plan Linked Notes?

There are two main reasons why you may not be able to write Service Plan Linked Notes:

  There is no Effective Date on the most recent plan - In order to write Service Plan Linked Notes, there must be an Effective Date on the most recent plan.

  Too much time has passed since the Effective Date - If an Effective Date has been entered on the most recent plan but Service Plan Linked Notes still cannot be written, it may be due to the amount of time that has passed since that Effective Date.  Within the PlanBuilder, each plan template has the option for setting an Amount of time after the effective date that a service plan linked note can be entered (located on the Plan Details page).  When the contact date of the Service Plan Linked Note falls outside of the range set in this field, a "Sorry, No Match Found" error message is displayed during the plan selection step of the note writing process.

Why haven't I been notified that a plan is ready to be e-signed?

In order for you to receive a notification that signing is signing available for a plan, the following must be true:

  You must have been selected (manually or by default) as an employee whose signature is required on the plan.

  The plan's signatures page must have been saved in data entry mode at least once.

  If there is more than one signing order level configured for the plan, any signing requirements for the level(s) prior to the one in which you are selected must have been met.  (If  the requirements have not yet been met the plan will display "Other Signatures Required Before Signing is Available" instead of a signing link.)

 Why isn't the information entered into a client's Immediate Needs Plan displaying on his/her Initial Plan?

Data entered in Immediate Needs Plans does not carry over to/display on Initial Plans.  Immediate Needs Plans are intended to be short-term and not part of a client's longer term plan.

Why wasn't I given the option of copying plan data from a previous plan when scheduling a new plan?

The Copy Data option provided when scheduling a review is only made available if the last plan has a due date that is prior to the due date of the new plan.

No, each plan will need to be created in the Plans and Reviews feature manually.

Will discontinuing a goal also discontinue the objectives below it?

Yes.  The discontinue tool discontinues not only the plan component for which it is used, but the items beneath that component; for example, discontinuing an objective will also discontinue the related interventions, and discontinuing a goal will also discontinue both the related objectives and interventions.
