Viewing a Face Sheet Report

A read-only report version of client face sheets is accessible from within the Reports module.  To view this type of face sheet report, complete the following steps:

  TIP: Face sheets can also be accessed in read-only report mode by first opening them in data entry mode and then clicking Printable Form at the bottom of the page.

    1. From the navigation bar, click Reports (or the Reports icon).  The Reports index page is displayed.

    1. Click the Program drop-down arrow and select the program for which a face sheet report is to be viewed.
    2. If you previously bookmarked the Face Sheet Report it is displayed on the reports index by default; otherwise, from the left-hand menu bar, click All under "Show Reports."
    3. Use the navigation features in the bottom-right corner of the reports index to locate Face Sheet Report, or enter "face sheet" in the Search field in the upper-right corner.
    4. Click Face Sheet Report from the reports index. The Program Profile / Face Sheet Report Selections page is displayed.

    1. To limit report selection by worker (case manager), click the Worker drop-down arrow and select the worker.  The default value is "All Workers."
    2. Click the Client drop-down arrow and select a client to view his or her face sheet report. The default value is "All Clients."

  TIP: If the face sheet to be viewed is for a former (discharged) client, first click Roster Archives beneath the client selection option.

  TIP: If a specific worker was selected in step 6, select "Worker Caseload" to view the face sheet reports of all consumers on that worker's caseload. 

    1. If "All Clients" or "Worker Caseload" was selected in step 7, type a date in the Modified Since field (using mm/dd/yyyy format) to view only those face sheets modified after the date specified.  The default value is six months prior to today's date.

  NOTE: This option is not applicable when a specific client was selected in step 7.

    1. Click SHOW INFO.  The face sheet report is displayed on the Client Face Sheet page, providing a read-only report version of the client's face sheet.  If more than one face sheet met the report selection criteria only the first is displayed initially; NEXT FaceSheet and PREV FaceSheet buttons are displayed at the bottom of the Client Face Sheet page to navigate through all available face sheet reports.
