Viewing a Client History Report

To view a Client History Report, complete the following steps:

  TIP:  The following instructions detail use of the Client History Report from within the Client Search tool.  It is also accessible from within the Reports module where it can be bookmarked for easy access.

    1. From the navigation bar, click in the Client Search box (or click the Search icon).  The Advanced Search window is displayed.

    1. Search for the client for whom the client history report is to be viewed by entering identifying information in one or more of the fields in this window.

  NOTE:  The available search fields may vary from those shown here.   

    1. Click the Number of Results drop-down arrow and select the number of client matches to be included in the Search results. 
    2. Click Search.  The Client Search Results page is displayed.  

    1. This page lists those clients with AWARDS records for whom all or part of the search criteria entered on the previous page are a match.
    2. Click the Client History Report icon to the left of the client for whom the report is to be viewed.

  NOTE:  The Client History Report icon is available for clients with a processed intake or room reservation, and/or a processed admission, in a program to which you have chart access.  It is not available for unprocessed referrals.

    1. The Client History Report Settings page is displayed. 

    1. The client history report includes only those "events" that take place within the date range specified on this page.  By default, the date range begins with the date of the client's first screening in the agency, or his or her initial intake date, whichever is earlier.  It ends with the date 90 days after the client's last discharge out of the agency, or 90 days after the current date if the client is presently enrolled in an agency program.  Make changes to the values in the Start and End date fields as necessary.

  NOTE:  The default start and end dates are both in relation to your chart access permissions.  For example, if a client was first admitted to a program to which you do not have chart access, and was later admitted to a program to which you do have chart access, the start date would default to the screening date for the second program (or the intake date if earlier), even though it is the more recent of the two screening dates.  

    1. Click CONTINUE.  The report is displayed on the Client History Report page.  For detailed information on the contents of this report, please see the Client History Report Details.

  TIP:  To export the report information to Microsoft Excel for further analysis, click the Excel File link at the top of the report page.  A PHI Download Alert is displayed, reminding you of your obligation to properly handle any confidential data and/or protected health information (PHI) within the download in accordance with related government regulations and agency policies.  After reading the alert, click OK to acknowledge it and proceed with the export, or Cancel to return to the previous page.

    1. To view the client chart record associated with a specific event in the events list portion of the report, click the name of that record from the event type column.  The contents of the record are displayed in read-only report format in a new AWARDS window.

  NOTE:  Client chart record links are only available from this report for records in modules to which you have access.

    1. To return to the client history report after viewing a record, click Client History Report at the bottom of that record.

The process of viewing a client history report is now complete.
