Support Services Contacts Fields / Options

Below is an alphabetical list of the fields and options located on the Support Services Contacts Information data entry page.  To jump directly to the information for a specific field or option, press <Ctrl+F> to access your browser's search field and enter the name of the field/option in question.

  NOTE: A gear icon  next to a field or option in this list indicates that it is either optional (and therefore not automatically available in all AWARDS databases), or that some aspect of it (for example drop-down selections) is configurable.  For more information on the ways in which this data entry page can be configured, see the Support Service Contacts portions of the Face Sheet - Implementation Request Form. To request changes detailed there, complete the form and submit it to [email protected].

Address

In this field, type or make changes to the address of the agency for which the support services contact works.

Agency

In this field, type or make changes to the name of the agency for which the support services contact works.

As Of | End Date

In these fields type or make changes to the date on which the consumer's relationship with the support services contact began and, if applicable, the date on which the relationship ended (both using mm/dd/yyyy format).

  TIP: When an end date is entered for a support services contact and the record is saved, the contact is marked as "discontinued" and can be overwritten.  When overwriting, delete the end date.  When that date is deleted, the contact history information is not saved.

Comments

In this text box type or make changes to any comments or notes regarding the support services contact.

Consent Type | Date Signed / Not Signed | Consent Expiration

To record or make changes to the consent information for this support services contact, configure the following fields and options:

  Consent Type - Click this drop-down arrow and select the type of consent relevant to this support services contact.

  Date Signed / Not Signed - In this field, type or make changes to the date on which consent was given or denied (using mm/dd/yyyy format), or select the date using the date picker drop-down arrow.

  Consent Expiration - In this field, type or make changes to the date on which consent expires (using mm/dd/yyyy format), or select the date using the date picker drop-down arrow.  This field cannot be left blank.  If an expiration date is not manually entered and the support services contact information is saved, the expiration date defaults to one year from the date signed/not signed.

Consents information from this portion of the support services contact record is compiled into the Consent Form accessed using the Medical module Consent Form feature.  For more information see Consents Form.

Email

In this field, type or make changes to the email address at which the support services contact can be reached.

NYS License | Authorization Dates

If support services contact information is being entered for a CR program consumer's Authorizing Psychiatrist or Physician, it is necessary to type his or her license number in the NYS License field, and to enter dates (using mm/dd/yy format) in the Next Authorization Due, Last Authorization Due, and Last Authorization Done fields.

  TIP: For detailed information on the authorizing psychiatrist/physician, click the Look Up License link next to the Authorizing Psychiatrist/Physician field.  When clicked, a search is performed on the web site for the Office of the Professions at the New York State Education Department.  A list of those professionals whose name matches that in the Authorizing Psychiatrist field is then displayed, at which time you access more detailed license information about the professional.

Phone

Click the drop-down arrows and select "Work," "Home," "Cell," "Beep," "Fax," or "TTY" to indicate the phone number to be entered for the support services contact.  Type the phone number(s) in the corresponding fields (using (nnn) nnn-nnnn format, followed by an extension if applicable).  Up to two numbers can be entered for each contact.

Role / Person

In this field, type or make changes to the name of the support services contact; for example, when adding or updating contact information for a therapist, type his or her name in the Therapist field.

  TIP: When entering an additional contact, both the contact's role and name must be entered, rather than only the name as must be entered for pre-defined support services contacts.  When an additional contact is entered and the support services contact record is saved, another blank additional contact will be available the next time the support services contacts record is opened in data entry mode.  In this way, unlimited additional contacts may be entered.
