The Medical module Consents Form feature is optional database enhancement used to generate and print consent for release of information forms.

  NOTE: Consent information is only collected for contacts and support services contacts if an optional consents feature has been turned on in your AWARDS database.  If you don't see the consent options during contact and support services contact data entry and would like to, please download and complete the Contacts portion of the Face Sheet - Implementation Request Form, and then submit it to [email protected].

Required Permissions

Because the Consents Form feature is optional, it is not automatically available in all databases.  When requested (by contacting the Help Desk) it is turned on behind-the-scenes in AWARDS by Foothold Technology, and is available to users with the following permissions:

Program Chart Access
Display Any Chart Records Buttons
Display Chart Records Medical Button

  NOTE: Permissions are assigned using the Permissions Maintenance feature.  If you do not have access to that feature and need a permission listed here, please contact your supervisor or your local Help Desk for assistance.
