Below is an alphabetical list of fields/options you may encounter on the Discharge Information page.  Keep in mind that the discharge form is configurable based on program settings, as well as using preferences that can be set by the Help Desk behind-the-scenes in AWARDS.  As a result, the version of the discharge form you see may not include all of the options listed below.  Additionally, the options in selection lists may vary from one program type to another.

  TIP: To review information on a specific field or option, scroll through the list below, or press <Ctrl+F> to access your browser's search feature and enter the name of the field/option in question.

Alerts

In the discharge note Alerts text box, type or make changes to a list of any risk factors for the consumer, including whether he/she is a danger to himself/herself and/or others, CPL status, physical health conditions and/or needs, and allergies.

  TIP: The disposition note, including the alerts, can be completed at a later time using the Services - Individual module's Progress Notes feature. For more information, see Entering a Progress Note.

Anonymous Summary

In the Anonymous Summary text box, type or make changes to a brief explanation (up to 3 lines) of why the discharge is occurring.  Do not use names in this explanation; only pronouns such as "he" or "she."

Authorization for Follow Up Info

In this portion of the form enter a name of contact, relationship to client, address of contact, and phone of contact.

Classification

Click the Classification drop-down arrow and select the discharge classification category.  Available options will vary from program type to program type.

Counselor / Supervisor

The discharge form contains read-only information on the service coordinator assigned to the client at the time of the discharge, as determined by assignments made using the Services - Individual module's Service Coordinators feature.  It also contains read-only information on the work supervisor for the service coordinator, as determined by assignments made using the Human Resources module Staff Information feature. 

  TIP: Service coordinator assignments cannot be changed after a client is discharged.  As a result, if the service coordinator information is incorrect on the discharge form, you will need to contact the Help Desk for assistance.  Please provide them with the program name, client name, discharge date, correct service coordinator, and the date on which the coordinator was assigned to that client so that the corrections can be made for you behind the scenes.

  TIP: Supervisor assignments can be changed from within the Human Resources module by adjusting the work supervisor assignment within the coordinator's staff information record.  (For more information, see Updating a Staff Information Record.)  Please keep in mind that the discharge records contain  information on the supervisor at the time of discharge, so any changes made after the fact will not be reflected there.  In order for the information to be changed on the discharge record itself, you will need to contact the Help Desk and provide them with the program name, client name, discharge date, and the date on which the coordinator was assigned to the supervisor so that the corrections can be made for you behind the scenes.

Destination

Click the Destination drop-down arrow and select the type of residence to which this consumer is moving.

  NOTE: This option is only available when discharging a consumer from a housing program or when updating the discharge record of a consumer discharged from a housing program.

Discharge Date

Click the Discharge Date drop-down arrow and select the date on which the consumer was discharged.  The default selection is today's date.  Other options in the selection list are based on the current date; specifically, if today's date is before the 16th of the month, the date drop-down list goes back to the first of the previous month.  If today's date is after the 16th of the month, the date drop-down list goes back to the first of the current month.

  TIP: Users with the Startup Period Backdating permission must type in a discharge date (using mm/dd/yy format) rather than select it from a drop-down list.

Discharge Summary

In the discharge note Discharge Summary text box, type or make changes to a brief discharge summary, including information such as the consumer's new address/placement, contact person, and follow-up arrangements.

The information entered in this text box is saved as a progress note section for the selected consumer.

  TIP: The disposition note, including the discharge summary, can be completed at a later time using the Services - Individual module's Progress Notes feature. For more information, see Entering a Progress Note.

Discharged To

In the Discharged To field, type or make changes to a brief description of the type of housing or program to which the consumer is being discharged.

Disposition Statement

In the discharge note Disposition Statement text box, type or make changes to the disposition statement, including the reason for closing/termination, whether it was planned/unplanned, and any factors contributing toward success or failure.

The information entered in this text box is saved as a progress note section for the selected consumer.

Housing Status at Discharge

Click the Housing Status at Discharge drop-down arrow and select the consumer's housing status at the time of his or her discharge.  

Income Sources

Under Income Sources check-off any income sources for the consumer at the time of his/her discharge and enter dollar amounts as appropriate.  Default values may be present if income source records have been entered for the consumer in Entitlements.

Monthly Income at Discharge

Click the Monthly Income at Discharge drop-down arrow and select the category that includes the amount of the client's monthly income at the time of his/her discharge.

New Residence County

Click the New Residence County drop-down arrow and select the county in which the consumer's new residence is located.

  NOTE: This option is only available when discharging a consumer from a housing program or when updating the discharge record of a consumer discharged from a housing program; however, it is not available if the consumer is moving to another agency housing program.

Non-Cash Benefits

Under Non-Cash Benefits check-off all non-cash benefits for the consumer at the time of his/her discharge.

OASAS Fields

These fields/options are unique to OASAS programs.  Configure each as needed to record the consumer's education, living situation, treatment history, substance abuse information, tobacco use, discharge status, referrals, medications used during treatment, domestic violence information, goal achievement evaluation detail, and more.

  NOTE: The OASAS portion of the discharge form includes read-only treatment visits and counseling sessions information.  When reviewing that information keep in mind that the Total Treatment Visits on the discharge form is a calculated count for OASAS.  As per the OASAS PAS 45, it is a count of the days on which the client received treatment.  It is calculated through face to face contacts log records, face to face progress notes, and program group activities for which the client's attendance is recorded.  The count of individual visits and group visits may add up to the number of total treatment visits; however, that will not always be the case.  Group and individual visits counts are for all sessions greater than 30 minutes, regardless of how many there are on a given day.  The treatment visits though only count 1 session per day, per the OASAS rule that total treatment visits cannot exceed the amount of days in the program.

Outcome Category

Click the Outcome Category drop-down arrow and select the discharge outcome category.  Available options will vary from program type to program type.

Primary Income Source at Discharge

Click the Primary Income Source at Discharge drop-down arrow and select the primary source of the consumer's monthly income at the time of his/her discharge.

Primary Referral / Secondary Referral

Click one or more of the referral drop-down arrows and select the type of program to which the consumer has been referred.

Reason for Discharge

Click the Reason for Discharge drop-down arrow and select the reason for the consumer's discharge.  Available options will vary from program type to program type.

Service Course

In the discharge note Service Course text box, type or make changes to a brief summary of the consumer's service course, new problems elicited, and any changes in the consumer's family situation, the consumer's prevocational activities, and his or her social functioning, environment, physical health, and/or mental health.
