Working with VPAC Assessments

VPAC Assessments are built into AWARDS in two distinct components, for a two-step workflow:

  Step 1. Filling Out a VPAC Assessment

  Step 2. Reviewing VPAC Results & Recommendations

Both of these steps are comprised of working with FormBuilder forms.  As a result, the overall assessment process has a similar look, feel, and workflow as you may have with any other FormBuilder form you may be using.  By default VPAC forms are located within the Services - Individual module in AWARDS, where they can be found under "Forms" in the module's fly-out menu.  

Keep in mind though, that when your agency subscribed to this add-on service they may have chosen to locate this feature elsewhere in AWARDS.  Regardless of the point of access, the overall functionality and how it is worked with are the same.

IMPORTANT! VPAC Assessments are intended to provide diagnosis recommendations only, and are intended for use with individuals 13 years or older.

Step 1: Filling Out an Assessment

To fill out a VPAC assessment (or to make changes to an existing, unsigned assessment), complete the following steps from the AWARDS Home screen:

    1. Click Charts from the left-hand menu, and then click Services - Individual.  The Individual Services fly-out menu is displayed.
    2. Click Forms, and then click VPAC Assessment.  The VPAC Assessment Menu page is displayed.

    1. Click the Client drop-down arrow and select the client for whom an assessment is to be filled out (or for whom an existing assessment is to be worked with, if needed).

  TIP: If working with a former (discharged) client, first click Roster Archives beneath the client selection option.

    1. By default the last three months worth of assessments will be displayed in an index during the data entry process.  If necessary, make changes to that default using the Date Range fields on this page.  (If you will be working with an existing assessment, be sure that its date falls within this range.)
    2. Click CONTINUE.  The assessments index is displayed for the selected client, and includes any existing assessments from the date range specified in step 4, along with the option to add a new assessment.
    3. To fill in a new assessment, click Create New VPAC Assessment.  (Otherwise, click the existing assessment to be opened and worked with.)  The VPAC Assessment page is displayed.
    4. The assessment form is comprised of introductory/explanatory text, dynamic sections of information pulled in from the client's AWARDS chart, and then Level 1 and Level 2 sections for each of the twelve diagnosis areas evaluated by the assessment.
    5. If necessary, use the available Update buttons to access and adjust any of the client's chart information.
    6. Next, for each of the twelve diagnosis areas on the assessment, do the following:

a)  Click UPDATE to open that portion of the form in data entry mode.

b)  Work with the client to answer the questions in Level 1

c) Click SAVE. The full assessment page is re-displayed.

d) If the automatically calculated score for Level 1 of the diagnosis area you just filled out is three or more, click UPDATE to re-open that portion of the form. (If the score for Level 1 is less than three, Level 2 does not need to be completed. Move to the next diagnosis area or proceed with step 10.)

e)  Work with the client to answer the questions in Level 2.

f)  Click SAVE.

    1. When the assessment has been completely filled in, scroll to the bottom and click Update Signature.  Use the e-signing options provided to assist the client with signing the form to attest to its content and lock it from further edits.

The process of filling in the VPAC Assessment is now complete.  Continue with Step 2: Reviewing Results & Recommendations.

 Step 2: Reviewing Results & Recommendations

To review the results and recommendations for a VPAC assessment that has already been filled out, complete the following steps from the AWARDS Home screen:

    1. Click Charts from the left-hand menu, and then click Services - Individual.  The Individual Services fly-out menu is displayed.
    2. Click Forms, and then click VPAC Results & Recommendations.  The VPAC Results & Recommendations Menu page is displayed.

    1. Click the Client drop-down arrow and select the client for whom results and recommendations are to be viewed.

  TIP: If working with a former (discharged) client, first click Roster Archives beneath the client selection option.

    1. The date range on this page defaults to the last three months.  If necessary, make changes to that default using the Date Range fields so that the date of the assessment for which results are to be viewed fall within this range.
    2. Click CONTINUE.  The results and recommendations index is displayed for the selected client, and includes any previously completed results within the date range specified in step 4.
    3. Click Create New VPAC Results & Recommendations.  The results and recommendations page is displayed, containing instructions and summary detail (automatically pulled in from the assessment you just completed) along with the scores from the assessment.  Based on those scores the results and recommendations page will advise whether treatment is suggested.
    4. If necessary, use the available Update buttons to access and adjust any of the results information.  Click Save on the data entry page that is then displayed once you have completed your work.
    5. When all work with the form is complete, scroll to the bottom and click Update Signature.  Use the e-signing options provided to assist the client with signing the form to attest to its content and lock it from further edits.

The process of reviewing results and recommendations is now complete.
