Below is an alphabetical list of the fields and options available during incident review data entry. To jump directly to the information for a specific field or option, press <Ctrl+F> to access your browser's search field and enter the name of the field/option in question.
Agency Deputy Director Recommendations
In the Agency Deputy Director Recommendations text box, type or make changes to any recommendations made by the deputy director of the agency regarding this incident.
The Incident Type selection defaults to the type chosen on the initial incident report. If necessary, click this drop-down arrow and select the correct incident type.
The number in the Internal ID # field defaults to that entered on the initial incident report. Make changes to that number as necessary.
Configure the following fields and options to enter or update the follow-up recommendations made regarding this incident:
Target Date - In this field, type or make changes to the target date for implementation of the corresponding recommendation.
Implementation Date - In this field, type or make changes to the date on which the recommendation was implemented, if applicable (using mm/dd/yy format).
Recommendation - In this text box, type or make changes to a description of the recommendation.
Up to two recommendations can be entered at one time. When both recommendations are entered and the form is updated, space for two more recommendations is available.
Review Close Date
In the Review Close Date field, type the date on which the review process for this incident was closed (using mm/dd/yy format). Once this date is entered and the form is updated, no additional changes may be made to the review form.
Review Committee Assessment / Findings of Action Taken
In the Review Committee Assessment / Findings of Action Taken text box, type or make changes to any information on the review committee's assessment of the incident and/or their findings regarding action taken.
Review Committee Comments or Questions
In the Review Committee Comments or Questions text box, type or make changes to any comments or questions posed by the review committee.
Signed Report In Date
In the Signed Report In Date field, type the date on which the incident review was opened (using mm/dd/yy format).
