To print a consent for release of information form, complete the following steps from the AWARDS Home screen:

1. Click the Program drop-down arrow in the upper right-hand corner of the page and select the program associated with the client for whom a consents form is to be printed.
2. Click Charts from the left-hand menu, and then click Medical.  The Medical fly-out menu is displayed.
3. Click Consents Form.  The Consents Form Client Selection page is displayed.

4. Click the Client drop-down arrow and select the client for whom the consents form is to be printed.

  TIP: If the consent form to be printed is for a former (discharged) client, first check Roster Archives beneath the client selection option.

5. Click SHOW FORM.  The consents form is displayed on the Consent for Release of Information page.
6. Use your web browser's Print option to print the form.

The process of printing a consents form is now complete.
