Where does the consent form information come from?  Can I add to it?

Where does the consent information information come from?  Can I add to it?

The consents information is pulled from contact information entered on the client's face sheet and within the Contacts feature in the Medical module if consents information has been entered for those contacts.  It is also pulled from the support service contacts information entered on the face sheet and within the Support Services Contacts feature in the Medical module when consents information has been entered for a contact there.  Beneath this information on the consent form are blank lines in which additional consents information can be entered.

  NOTE: Consent information is only collected for contacts and support services contacts if an optional consents feature has been turned on in your AWARDS database.  If you don't see the consent options  during contact and support services contact data entry and would like to, please download and complete the Contacts portion of the Face Sheet Implementation Request Form, and then submit it to [email protected].
