Entering / Updating Assessment Data

IMPORTANT! In order to enter or update assessment data, the following criteria must be met:

An assessment instrument or FormBuilder form MUST exist for the program for which the assessment data entry is to be done.  This assessment instrument/form must be set up for the assessment type for which data is to be entered or updated, and must have a deployment date prior to the charting timetable assessment event due date.

The client for whom the assessment data is to be entered or updated MUST have a charting timetable record for the same assessment type as the assessment instrument / form.

To enter or update assessment data for a charting timetable event, complete the following steps from the AWARDS Home screen:

    1. Click the Program drop-down arrow and select the program associated with the client for whom assessment data is to be entered or updated.
    2. Click Charts from the left-hand menu, and then click Services - Individual.  The Individual Services fly-out menu is displayed.
    3. Click Assessment Data.  The Assessment Selection page is displayed.

    1. Click the Client drop-down arrow and select the client for whom the assessment data is to be entered or updated.

  TIP: If the assessment data to be worked with is for a former (discharged) client, first click the Roster Archives checkbox beneath the client selection option.

    1. Click the Assessment Type drop-down arrow and select the type of event for which assessment data is to be entered or updated.  The default value is "All Types."
    2. Verify that the Database option is set to "Data Entry."
    3. Only those charting timetable events with due dates within the date range in the From and To fields on this page will be available for assessment data entry.  Make changes to the default date range as necessary (using mm/dd/yyyy format).
    4. Click CONTINUE.  The Assessment Timetable Data Entry page is displayed.

This page contains a list of charting timetable events meeting the selection criteria entered on the previous page.

    1. Click the Open? radio button next to the assessment for which data is to be entered or updated.

  TIP: If the assessment you wanted to work with is not yet scheduled, you can use the Charting Timetable shortcut button located on this page to access the Services - Individual module Charting Timetable feature.  

    1. Click OPEN.  The assessment form corresponding to the selected assessment type is displayed on the Assessment Form page.
    2. Make changes or additions to the information on this page as necessary.
    3. Click UPDATE.  The assessment information is saved and a read-only report version of the assessment form is displayed.

The process of entering or updating assessment data is now complete.
