Entering / Updating an Admission Note

To enter or update an admission note, complete the following steps from the AWARDS Home screen:

    1. Click the Program drop-down arrow and select the program associated with the consumer for whom the admission note is to be entered or updated.
    2. Click Charts from the left-hand menu, and then click Services - Individual.  The Individual Services menu fly-out is displayed.
    3. Click Service Plans.  The Service Plan Consumer Selection page is displayed.
    4. Click the Consumer drop-down arrow and select the consumer for whom the admission note is to be entered or updated.

  TIP: If the admission note is to be entered or updated for a former (discharged) consumer, first click the Roster Archives checkbox.

    1. Click CONTINUE.  The Individual Service Plan Index page is displayed.  This page contains a list of existing plans scheduled for the selected consumer.

If an admission note schedule record must be entered or updated, continue with step 6.  If an existing admission note is to be worked on and it is not necessary to enter or update a schedule record, continue with step 9.

  NOTE: In most cases an admission note will have been scheduled for the consumer during the intake/admission process.

    1. Click Update Schedule.  The Individual Service Plan Review Schedule page is displayed.
    2. To update an existing schedule record, make changes to the information in the review schedule fields and options as necessary.  To enter a new schedule record, configure the blank NEXT or BACK FILL review schedule fields and options.  Available fields and options are:

  Due Date - In this field, type the date on which the admission note must be done (using mm/dd/yy format).

  Plan Type - Click this drop-down arrow and select "Admission Note."

  Reviewer - Click this drop-down arrow and select the worker responsible for completing the admission note.

  Done - Click this drop-down arrow and select "Yes" or "No" to indicate whether the admission note has been completed.

  Done Date - If "Yes" was selected for the Done option, In this field, type the date on which the admission note was completed (using mm/dd/yy format).

    1. Click CONTINUE.  The schedule record is saved and the updated Individual Service Plan Index page is displayed.
    2. Click the Selected radio button next to the admission note.
    3. Click CONTINUE.  The Admission Note page is displayed.
    4. If necessary, in the Initial Plan Due Date field, type the date on which the corresponding initial plan must be completed (using mm/dd/yy format).
    5. Enter information as needed in the Alerts portion of the admission note using the Alerts and Alerts, Barriers, Risks fields and options.

  NOTE: If "Physical/Sexual Abuse or Victim" is selected as an Alert, click either the Victim or Abuser radio button.  If "Other" is selected, enter the type of alert in the field provided.

    1. In the Initial Utilization of Services Justification text box in the Reason for Admission portion of the admission note, type or make changes to information on the reason for consumer admission and justification for service utilization.
    2. Make changes or additions to the information in the Diagnoses portion of the admission note using the available fields and options.
    3. In the Allergies field, type or make changes to information on any allergies the consumer has.
    4. Set the 28 day immediate needs and initial plan by configuring or making changes to the information in the following fields and options:

  Needs / Add Needs - In these text boxes, type the immediate needs and plan objective.

  Service / Plan - Click the drop-down arrow and select the service type for the corresponding need/objective.  In the corresponding text box, type a detailed description of the service.

  Delete Pair - Click this checkbox to delete the need and service/plan pair.  The pair is deleted after UPDATE is clicked.

  NOTE: This option is only available after the need and service/plan pair has been saved at least once and no progress notes have been written against it.

Repeat as necessary.  Up to four need and service/plan pairs can be entered.

    1. Click UPDATE.  The admission note is saved, and need and service/plan pairs marked for deletion are deleted, and a read-only report version of the admission note service plan is displayed on the Admission Note page.

  TIP: To make additional changes to the admission note at this time, such as adding need and service/plan pairs, or editing existing admission note information, click DATA ENTRY to return to the Admission Note update page.

The process of entering or updating an admission note is now complete.
