Entering / Updating a Plan Review

To enter or update a periodic service plan review, complete the following steps from the AWARDS Home screen:

    1. Click the Program drop-down arrow and select the program associated with the consumer for whom the plan review is to be entered or updated.
    2. Click Charts from the left-hand menu, and then click Services - Individual.  The Individual Services menu fly-out is displayed.
    3. Click Service Plans.  The Service Plan Consumer Selection page is displayed.
    4. Click the Consumer drop-down arrow and select the consumer for whom the plan review is to be entered or updated.

  TIP: If the plan review is to be entered or updated for a former (discharged) consumer, first click the Roster Archives checkbox.

    1. Click CONTINUE.  The Individual Service Plan Index page is displayed.  This page contains a list of existing plans scheduled for the selected consumer.

If a plan review schedule record must be entered or updated, continue with step 6.  If an existing plan review is to be worked on and it is not necessary to enter or update a schedule record, continue with step 9.

  NOTE: The first periodic service plan review is automatically scheduled for the consumer when his or her initial plan is opened and updated for the first time. Subsequent service plan reviews are automatically scheduled for a consumer when the previous review for that consumer is opened and updated for the first time.

    1. Click Update Schedule.  The Individual Service Plan Review Schedule page is displayed.
    2. To update an existing schedule record, make changes to the information in the review schedule fields and options as necessary.  To enter a new schedule record, configure the blank NEXT or BACK FILL review schedule fields and options.  Available fields and options are:

  Due Date - In this field, type the date on which the review must be done (using mm/dd/yy format).

  Plan Type - Click this drop-down arrow and select "Service Plan Review."

  Reviewer - Click this drop-down arrow and select the worker responsible for completing the review.

  Done - Click this drop-down arrow and select "Yes" or "No" to indicate whether the review has been completed.

  Done Date - If "Yes" was selected for the Done option, In this field, type the date on which the review was completed (using mm/dd/yy format).

    1. Click CONTINUE.  The schedule record is saved, and the updated Individual Service Plan Index page is displayed.
    2. Click the Selected radio button next to the service plan review to be worked on.
    3. Click CONTINUE.  The Periodic Service Plan Review page is displayed.
    4. If necessary in the Next Review Due Date field, make changes to the date on which the next periodic review must be done for this consumer.  The default value is the service plan date plus 90 days.
    5. Make changes to the functional assessment fields, options, and/or text boxes as necessary.
    6. Type or make changes to the information in the Consumer Strengths, Consumer Limitations, Staff Responsible for Service Delivery, and Criteria for Discharge text boxes.
    7. Click UPDATE & CONTINUE.  The Individual Service Plan Goals, Objectives and Methods/Services page is displayed.
    8. If applicable, in the Completion Date field, type the date on which the service plan review was completed.

  NOTE: A completion date cannot be entered until a review of objective progress has been completed.

    1. Review the progress on all existing objectives/methods by configuring the following fields and options:

  TIP: Service plan changes such as the addition of new objectives cannot be made until this step is complete.

  Objective Target Date - If necessary, make changes to the target date for the corresponding objective (using mm/dd/yy format).

  Review of Progress / Objective Achievement - In the text box provided, describe the progress toward achievement of the objective.

  Objective Outcome / Method Outcome - Click the drop-down arrow and make an outcome selection for each objective and method.

  Consumer Comments - In the text box provided, type any consumer comments.

Continue this process through all existing objectives and methods.

    1. Click UPDATE.  The service plan review information is saved, and a read-only report version of the service plan is displayed on the Periodic Service Plan Review page.
    2. To add new objective/methods and/or to continue to revise the service plan, click DATA ENTRY.  The Individual Service Plan Goals, Objectives and Methods/Services update page is re-displayed.
    3. To further revise existing objectives/methods, configure the following fields and options:

  Objective Target Date - If necessary, make changes to the target date for the corresponding objective (using mm/dd/yy format).

  Objective Outcome - Click the second Objective Outcome drop-down arrow and make a status selection for the objective.

  NOTE: If "Objective Revised" is selected, click the corresponding Revised Objective Category drop-down arrow and select the objective category, then type the objective in the corresponding text box to set the revised objective.

  Method Outcome - Click the second Method Outcome drop-down arrow and make a status selection for the method.

  NOTE: If "Method Revised" is selected, click the corresponding Method drop-down arrow and select the method type, then type the method in the corresponding text box.

    1. To add new objectives/methods follow the same steps necessary to add a new objective/method in an initial plan.  This includes the configuration of the following fields and options:  New Objective Start, Category, New Method or Add Method, and Method Start.  If necessary, objectives and methods can also be deleted using the Delete Objective and Delete Method options, respectively.

  NOTE: Delete Objective and Delete Method options are only available after the objective/methods have been saved at least once and no progress notes have been written against the objective and/or method.

    1. Click UPDATE.  The service plan review information is saved, any objectives or methods marked for deletion are removed, and a read-only report version of the service plan review is displayed on the Periodic Service Plan Review page.

  TIP: To make additional changes to the plan review at this time, such as adding additional objectives/methods or editing existing plan information, click DATA ENTRY to return to the Individual Service Plan Goals, Objectives and Methods/Services update page.

The process of entering or updating a service plan review is now complete.
