Below is an alphabetical list of the fields and options located on the hospital episodes data entry page. To jump directly to the information for a specific field or option, press <Ctrl+F> to access your browser's search field and enter the name of the field/option in question.
NOTE: A gear icon next to a field or option in this list indicates that it is either optional (and therefore not automatically in all AWARDS databases), or that some aspect of it (for example drop-down selections or its status as required/not required) is configurable. For assistance in configuring these items, please contact the Help Desk.
In the Admission Date field, type or make changes to the date on which the client was admitted to the selected facility (using mm/dd/yy format), or select the date using the available date picker drop-down.
In this text box, type or make changes to any comments pertaining to the hospital episode.
If applicable, in the Discharge Date field, type or make changes to the date on which the client was discharged from the selected facility (using mm/dd/yy format), or select the date using the available date picker drop-down.
Hospital / Facility
Click the Hospital/Facility drop-down arrow and select the hospital/facility for which the selected client's hospital episode record is being worked with.
NOTE: The hospitals/facilities listed in the drop-down selection list are those found in the AWARDS Hospitals list.
If the correct hospital/facility is not listed, scroll to the bottom of the list and select "Add New." A dialog box in which you can enter the new hospital/facility is now displayed.
Configure the fields and options on this box with the hospital/facility information, and then click Save to return to the hospital episode record. Hospitals/facilities added using this method are automatically added to the list maintained in the Hospitals feature, and will appear on the hospitals index there for editing.
TIP: Alternately, the new hospital/facility record can be created in the Hospitals feature prior to documenting the hospital episode.
Reason / Outcome
In the Reason/Outcome field, type or make changes to any notes regarding the reason for the hospital episode and/or its outcome.
NOTE: If you do not currently see the Reason/Outcome field and would like it to be made available, please contact the Help Desk for assistance.
Click the Treatment drop-down arrow and select the type of treatment the consumer received, or is currently receiving, at the selected hospital/facility. Default selection options are:
