Viewing a Progress Notes Report

To view a read-only progress notes report, complete the following steps:

  1. From the AWARDS Home screen, click Charts from the left-hand menu, and then click Services.  The Client Services Menu page is displayed.

  1. Click the Program drop-down arrow and select the program or program type for which the report to be viewed.
  2. If the report is to be viewed for a former (discharged) consumer, click the Roster Archives check box.
  3. Click Utilization Reports.  The Services Utilization Reports page is displayed.

  1. Click Progress Notes Report.  The Progress Notes Report Selections page is displayed.

  1. To limit report selection by note writer, click the Worker drop-down arrow and select the writer.  The default value is "All Workers."
  2. Click the Consumer drop-down arrow and select the consumer(s) for whom the report is to be viewed.  The default value is "All Consumers."

If a worker was selected in step 6, select "Writer Caseload" to view the progress notes report for all consumers assigned to that writer.

  1. Only those progress notes written for contacts occurring within the date range in the From and To fields on this page will be included in the report.  If necessary, make changes to the default date range (using mm/dd/yyyy format).
  2. Click the Sort drop-down arrow and make a selection to indicate the order in which the report information is to be displayed.  Available options are:
  1. Click RUN REPORT.  The Progress Notes Report is displayed on the Progress Notes page.

The contents of this read-only report are based on the selections made on the previous pages.
