Generating a Group Attendance Form

To generate a group attendance form containing the roster for a scheduled group, complete the following steps from the AWARDS Home screen:

    1. Click the Program drop-down arrow in the upper-right corner of the page and select the program for which the form is to be generated.
    2. Click Charts from the left-hand menu, and then click Services - Group.  The Group Services fly-out menu is displayed.
    3. Click Group Schedule Setup.  The Group Schedule Setup fly-out menu is displayed.
    4. Click Group Attendance Form. The Group Attendance Form Menu is displayed.

    1. Click the Activity drop-down arrow and select the activity for which the form is to be generated.
    2. By default the group attendance form is generated for the selected activity for today through the coming week.  Use the Date Range From and To fields on this page to adjust this default range as needed (using mm/dd/yyyy format or the available date pickers).  Keep in mind that the maximum date range is one month, and that only activities scheduled for the future will be included on the attendance form.
    3. Indicate how the form content is to be displayed by clicking one of the available Display Options radio buttons:

  Attendance Form - When selected the attendance form includes a list of enrolled group members with checkboxes next to each name where attendance can be noted prior to or after printing.  A signature slot is also included for each participant, several blank lines are provided for noting additional participants, and a staff signature slot is also provided beneath each group's attendance table.

  NOTE: The available checkboxes are for printing purposes only and any selections made will not be saved in AWARDS.

  Groups Linkage - When selected the attendance form includes a list of enrolled group members, along with the goals and services assigned to those group members using the Group Linkage feature in the Services - Group module's Service Plans feature.  A staff signature slot is also provided beneath the group's linkage table.  No attendance checkboxes, signature slots, or blank rows are included when using this display option.

    1. Click CONTINUE.  The group attendance form is generated and displayed on the Group Attendance Form page.  The contents of this page are based on the settings selected in the previous steps.
    2. At this time, use your browser's printing options or click the AWARDS Logo in the right-hand corner of the page to initiate the printing process.

  TIP: As a reminder, when using the "Attendance Form" display option you may first choose to use the participant attendance checkboxes prior to printing, or wait and check them off manually on the printed form. (Checkbox selections made directly in AWARDS are only for printing purposes and are not saved.)
