Generating / Printing a Hospitalization Referral Letter

To generate and print a hospitalization referral letter, complete the following steps from the AWARDS Home screen:

    1. Click the Program drop-down arrow in the upper right-hand corner of the page and select the program associated with the client to be worked with.
    2. Click Charts in the left-hand menu, and then click Hospital.  The Hospital fly-out menu is displayed.
    3. Click Hospitalization Referral Letter.  The Hospitalization Referral Letter Data Entry page is displayed.

    1. Click the Client drop-down arrow and select the client for whom the hospitalization referral letter is to be generated and printed.

  TIP: If the referral letter to be generated and printed is for a former (discharged) client, first click Roster Archives beneath the client selection list.

    1. Click CONTINUE.  The Hospitalization Referral Letter Editing Index page is displayed.

    1. Click the Treatment drop-down arrow and select the type of treatment for which the consumer is being referred.  Available options are "Psychiatric" and "Medical."
    2. Click the Create New Hospitalization Referral Letter or Edit Hospitalization Referral Letter radio button to indicate which task is to be performed.  

  NOTE: The Create New option is available when no hospitalization referral letters have been written today for the consumer by this worker.  The Edit option is available when a hospitalization referral letter has already been written today for the consumer by this worker. 

    1. Click CONTINUE.  The referral letter data entry page is displayed.
    2. Enter or update the text in the fields and text box on this page as necessary.  There are places provided for the entry of precipitating behavior, service coordinator contact, and degree of threat information (when the referral is psychiatric in nature).

  TIP: Each field on this page has a cue as to the information it should contain.  Before generating the letter, check each field carefully to make sure it contains the correct information and not simply a cue.

  NOTE: Text entered in the precipitating behavior text box is saved as a progress note section for the selected client.  This progress note section is available for viewing using the Services - Individual module Progress Notes feature in report mode.  For more information, see Viewing a Progress Notes Report.

    1. Click GENERATE LETTER.  The precipitating behavior information from the referral letter information is saved as a progress note section, and a read-only printable version of the letter is displayed.
    2. To print the letter, use your browser's print functionality and follow the instructions on the page to complete the printing process.

The process of generating and printing a hospitalization referral letter is now complete.
