Working with Pre-Discharge Status Information

To enter or update pre-discharge status information, complete the following steps from the AWARDS Home screen:

    1. Click the Program drop-down arrow in the upper right-hand corner of the page and select the program to be worked with.
    2. Click Census from the left-hand menu, and then click Discharge.  The Discharge menu fly-out is displayed.
    3. Click Pre-Discharge Status. The Pre-Discharge Status Data Entry Client Selection page is displayed.

    1. Click the Client drop-down arrow and select the client for whom the pre-discharge status information is to be worked with.
    2. Click CONTINUE.  The Pre-Discharge Status page is displayed.

This page contains previously entered pre-discharge status information for the selected client (if any), as well as a blank row for entering a new status.  Anticipated discharge information is also displayed if it was entered in the past.

    1. Make changes to any existing information as necessary, and/or enter new information by configuring the fields/options in the blank status record row as follows:

  Status - Click this drop-down arrow and select the client's pre-discharge status.

  Status Date - In this field, type the date (using mm/dd/yyyy format) on which the selected status first applied to the client. 

  End Date - In this field, type the end date for the selected status, if applicable.

  Respite / Visitation Site - Click this drop-down arrow and select one of the available programs, if applicable.  Once the pre-discharge status information has been saved at least once, this selection cannot be changed.

    1. In the Anticipated Discharge Date field, type or make changes to the client's anticipated date of discharge (using mm/dd/yyyy format), if applicable, and then click the corresponding to drop-down arrow and select the residential program to which the client will be discharged.  Select "Discharge from Agency Housing" from this list if the client is not expected to be discharged to another agency program.
    2. Click UPDATE.  The pre-discharge information is saved and the updated Pre-Discharge Status page is displayed.

The process of entering pre-discharge status information is now complete.
