Behaviors data entry takes place in the following two components:

Behavior Tracking

Below is an alphabetical list of the fields and options located in each of these components.  To jump directly to the information for a specific field or option, press <Ctrl+F> to access your browser's search field and enter the name of the field/option in question.

  NOTE: A gear icon  next to a field or option in this list indicates that some aspect of it for example drop-down selections or its status as required/not required) is configurable. To request changes to those items shown here as configurable, please contact the Help Desk for assistance.

Behaviors

Below is an alphabetical list of the fields and options located on the behaviors data entry page.  To jump directly to the information for a specific field or option, press <Ctrl+F> to access your browser's search field and enter the name of the field/option in question.

Antecedents

  NOTE: This option is only available when Type is set to "Target - Challenging" or "Target - Positive."

In this text box, type or make changes to any known antecedents (preceding circumstance, event, etc.) related to the behavior.

Baseline Frequency

  NOTE: This option is only available when Type is set to "Target - Challenging" or "Target - Positive."

Click this drop-down arrow and select the behavior's baseline frequency.  Default selections are "Low," "Medium," and "High."

Baseline Severity

  NOTE: This option is only available when Type is set to "Target - Challenging."

Click this drop-down arrow and select the behavior's baseline severity.  Default selections are "Mild," "Moderate," and "Severe."

Brief Description

In this field, type or make changes to a brief description of the target or replacement behavior, using 35 characters or less.

Detail

In this text box, type or make changes to a detailed description of the behavior.

End Date

In this field, type or make changes to the date on which the behavior ended, if applicable.  Use mm/dd/yyyy format, or select the date using the available date picker.

  TIP: The date entered cannot be prior to the Start Date or be a date in the future.

Interventions / Staff Actions

  NOTE: This option is only available when Type is set to "Target - Challenging" or "Target - Positive."

In this text box, type or make changes to detailed information on the interventions to be used for this behavior.

Reinforcement Instructions

  NOTE: This option is only available when Type is set to "Target - Challenging" or "Target - Positive."

In this text box, type or make changes to detailed reinforcement instructions for any positive behavior.

Related To

  NOTE: This option is only available when Type is set to "Replacement."

A checklist comprised of any existing behaviors for this individual that have a Type of "Target - Challenging."  Check off one or more of the available target behaviors to indicate which of them this replacement behavior is intending to target.  

  NOTE: If a target behavior selected here is deleted in the future, the "Related To" association will also be deleted from this record.

Start Date

In this field type or make changes to the date on which the behavior started, if applicable.  Use mm/dd/yyyy format, or select the date using the available date picker.  

  TIP: The date entered cannot be in the future.

Tracking Instructions

  NOTE: This option is only available when Type is set to "Target - Challenging" or "Target - Positive."

In this text box, type or make changes to instructions for the clinicians tracking this behavior.

Tracking Interval

Click this drop-down arrow and select the tracking interval for this behavior.  Default selections are "Per shift," "daily," "hourly," and "weekly."

Type

Click this drop-down arrow and select the type of behavior.  Available selections are "Replacement," "Target - Challenging," and "Target - Positive."

  TIP: Replacement behaviors are intended to be those that the clinician wants the individual to replace challenging target behaviors with. Target behaviors are those intended to be "targeted" to work on, either positive or challenging.

   Behavior Tracking

Below is an alphabetical list of the fields and options located on the behavior tracking data entry page.  To jump directly to the information for a specific field or option, press <Ctrl+F> to access your browser's search field and enter the name of the field/option in question.

# of Occurrences

In this field, type or make changes to the number of occurrences of this behavior during the tracking episode timeframe.  Enter "0" to reflect the absence of a behavior.

# of Reinforcements

In this field, type or make changes to the number of reinforcements for this behavior during the tracking episode timeframe.

Antecedents

In this text box, type or make changes to any antecedents (preceding circumstance, event, etc.) for the tracked episode.

Brief Description

In this text box, type or make changes to a description of the tracked episode.

Date

In this field, type or make changes to the tracking date.  Use mm/dd/yyyy format, or select the date using the available date picker.  The default value is today's date.  

  TIP: The date entered cannot be earlier than the Start Date of the associated behavior.

Duration (minutes)

In this field, type or make changes to the duration of the episode, in minutes.

Intervention(s) Used / Staff Actions

In this text box, type or make changes to detailed information on the interventions used during this tracking episode.

Reinforcements Used

In this text box, type or make changes to a description of any reinforcements used during the tracking episode.  For instance, if the associated behavior's Type is "Target - Challenging," this text box could be filled out if there is a reinforcement for the absence of a behavior.

Severity

  NOTE: This option is only available when the associated behavior's Type is "Target - Challenging."

Click this drop-down arrow and make a selection to indicate the behavior's severity.  Default selections are "Mild," "Moderate," and "Severe."

Start Time

In this field, type or make changes to the time of the behavior tracking using HH:MM:SS AM/PM format.  The default value is the current time.
