Program Status Fields / Options

Below is an alphabetical list of the fields and options located on the Program Status - Data Entry page.  To jump directly to the information for a specific field or option, press <Ctrl+F> to access your browser's search field and enter the name of the field/option in question. 

  NOTE: A gear icon  next to a field or option in this list indicates that it configurable and may vary from one program to another.  For more information on the ways in which this data entry page can be configured, see the Program Status portion of the Face Sheet - Implementation Request Form.  To request some or all of the configuration options detailed there, complete the form and submit it to [email protected].

Comments

In this field, enter any comments related to the status, as needed.

End Date

In this field, enter the date on which the status ended, if applicable, or click the drop-down arrow and select the date using the available date picker.

  TIP: The date entered must fall within a program history episode for the client in the selected program.  Future dates are not allowed.

IMPORTANT! If the start date entered falls within a previous program history episode for the client, an end date must be entered.  That end date must be prior to the client's discharge date for the program episode.

Include Form

If the program status record should include a FormBuilder form, click this drop-down arrow and select the appropriate form.  The page automatically refreshes to make the form available for immediate data entry.  Any information entered in the form will be saved when Save is clicked during the data entry process, will be displayed with the record in report mode, and will be available on the Program Status ReportBuilder.  The name of the included form will also be shown on the program status record index.

  NOTE: The form selection list is only available when forms created in the FormBuilder have been set with the "program status" location and have been made available to the selected program - AND - you are adding a new program status record, or working with an existing program status record for which a form has not yet been included.

  TIP: If you are working with an existing record for which there IS already an included form, the form selection list is replaced by a "Delete Form" option.  To remove the included form (if it has not been electronically signed), click the Delete Form button found there.  Once an included form has been removed, you can then include a different form if needed, or re-include the same form if you'd like to start it from scratch.

Start Date

In this field, enter the date on which the status went into effect, or click the drop-down arrow and select the date using the available date picker.

  TIP: The date entered must fall within a program history episode for the client in the selected program.  Future dates are not allowed.

Status

Click this drop-down arrow and select the client's status.
