Generating a Fee Agreement Form

To enter new money management information and generate a fee agreement form, complete the following steps:

  1. From the AWARDS Home screen, click Charts from the left-hand menu, and then click  Entitlements.  The Entitlements Menu page is displayed.

  1. Click the Program drop-down arrow and select the residential program associated with the consumer for whom the fee agreement form is to be generated.
  2. Click the Database drop-down arrow and select "Data Entry."
  3. To limit consumer selection by name, click the A-Z drop-down arrow and select the consumer's last name initial.  An asterisk (*) in this field will include all consumers in the selection process.
  4. Click Money Management.  The Money Management Menu page is displayed.

  1. Click the Consumer drop-down arrow and select the consumer for whom the fee agreement form is to be generated.
  2. Click one of the money management radio buttons to indicate the type of information to be displayed during the process of generating a new fee agreement form.  Available options are:
  1. Click CONTINUE.  The Money Management/Allowance Agreement Status Report page is displayed.

This page contains options for entering new money management information as well as a read-only list of the current and/or any past fee agreements/money management information .

  1. Click the Payee Status drop-down arrow and make a selection to indicate how the check from the benefit agency is made out, or how payments are made.  Available selections are:
  1. Click the Level drop-down arrow and make a selection to indicate how often payments of Personal Needs Allowances (PNA) are made by the agency to the consumer.  Available selections are:
  1. Click Update Log & Create Form.  The money management information is saved and the fee agreement form is displayed on the Fee Agreement page.
  2. Click Print Form to print the fee agreement form.  The Print dialog box is displayed.
  3. Configure the print options as necessary.
  4. Click OK.  The fee agreement form is printed.

The process of entering money management information and generating a fee agreement form is now complete.

To reprint this fee agreement form at any time, use the Money Management feature in report mode.  Do not use the steps provided above, as doing so will generate another form.  For more information, see Viewing a Money Management/Allowance Agreement Report.
